Position Highlights:

  • Responsible for helping to build and support a heart failure guideline directed medical therapy titration clinic providing both face-to-face and virtual services to heart failure patients with the goal of rapid medication titration and evaluation for device or advanced heart failure therapies if indicated.
  • Cover in our longitudinal heart failure and heart success clinics as needed providing care to patients who have advanced systolic heart failure and/or diastolic heart failure from ischemic, nonischemic, infiltrative, structural, congenital or various other cardiomyopathies
  • Work closely with a multidisciplinary team to provide comprehensive patient care in various outpatient clinical settings.
  • Responsible for helping to build and support a heart failure guideline directed medical therapy titration clinic.
  • Willingness to work five 8-hour shifts; M-F 8:30am-4:30pm.
  • No weekends, call or night shifts required.

Practice Highlights:

  • Multidisciplinary team comprised of physicians, APPs, nurse navigators, clinic nurses, clinical pharmacists, social workers, and various other support staff across 3 facilities.
  • SHVI Charlotte offers a full team of specialized heart failure providers and advanced heart failure services including:
  • Congestive heart failure
  • Left-sided heart failure, including systolic and diastolic failure.
  • Right-sided heart failure
  • Cardio-oncology
  • Infiltrative cardiomyopathies
  • Hypertrophic cardiomyopathy
  • Pulmonary hypertension
  • Full range of heart failure treatment options, including:
  • Heart transplat
  • Mechanical circulatory support, including LVADs, BiVADs, Impella, IABPs, ECMO
  • Total artificial heart
  • Minimally invasive surgery
  • Implantable devices and defibrillators
  • Medication therapy and management

Position Qualifications:

  • Board certified nurse practitioners and/or physician assistants specifically trained and proficient in providing cardiac services
  • Cardiology experience required; prior heart failure experience preferred
  • Master's Degree or Doctor of Nursing Practice Required. ACNP, AGACNP preferred based on population served.
  • * Must sit for and pass Certified Heart Failure Nurse (CHFN) examination within 2 years of beginning practice .
  • Master's Degree/Physician Assistant required.
  • Demonstrate appropriate critical thinking and interpersonal skills when working in a faced paced, high acuity work environment. Able to perform a thorough history and physical assessment.
  • NC/SC Licensure
